Written application: You may personally deliver a wet-signed copy of the KVKK Application Form along with an identity document, or via authorized representative with a notarized power of attorney, to Çınarlı Mah, 1572. Sk. No:33, 35170 Konak/İzmir, or send it via notary.

Electronic application: You may sign the KVKK Application Form with a "secure electronic signature" as defined in the Electronic Signature Law No. 5070 and send it from your email address registered in our systems to info@healthupmarketing.com. Additional verifications (such as sending a message to your registered email address or contacting you by phone) may be requested to confirm your identity and protect your rights. If the application is submitted by a third party on behalf of the data subject, the applicant must present a notarized special power of attorney granted by the data subject.
